Alisat Home Health Agency welcomes referrals from individuals, families, guardians, SSAs, case managers, hospital discharge planners, healthcare professionals, and community partners.

- Individuals with developmental disabilities who qualify for Ohio DODD services
- Older adults authorized for Ohio Department of Aging PASSPORT services
Referral Steps

- Tell us which services are being requested, the preferred schedule, location, transportation needs, and anticipated start date.
- With appropriate authorization, we coordinate with the individual, guardian, SSA or case manager to review the service plan and confirm availability.
- We introduce appropriate staff, establish clear communication, and prepare for a dependable start of care.
