DODD & ODA PASSPORT SERVICES

Two specialized programs. One commitment to excellent care.

Alisat Home Health Agency is an Ohio DODD-certified provider and an Ohio Department of Aging PASSPORT provider. We deliver dependable, person-centered support for individuals with developmental disabilities and older adults who want to remain safe, independent, and connected to their communities.

DODD services: Homemaker/Personal Care, HPC Transportation, Non-Medical Transportation, Ohio Shared Living, Residential Respite, Participant-Directed HPC, DD Waiver Nursing, Nursing Delegation, and Money Management.

ODA PASSPORT services: Personal Care, Homemaker Services, Waiver Nursing, Non-Medical Transportation, and community-based support according to each person’s authorized care plan.
